 Program RequirementsPlease fill out the application form to the left. We require that you upload, email or fax your cosmetology license and driver's license with your application. You may fill out the convenient online form, or you may send the form to sales@perfectlocks.com or fax it to 925.349.4050 once completed. There is no application fee required to submit your application. Our Partner Program has been designed to work for stylists, salons, and retailers. Once enrolled in the program, you will have immediate access to deeply discounted prices on top quality hair from Perfect Locks.
